What's Your Best Response to Management? "We Tried Six Sigma and it Didn't Work"

There's always pressure for continuous improvement but your organization is not sold on Six Sigma. In fact, they had previously tried using Six Sigma before your time and failed. What is your best response to management to initiate another Six Sigma project?

Things to think about:
- Every project, whether in success or failure should be formally closed with a Lessons Learned report. What does the document say about the causes of failure? If absent, what does that say about the system for project performance and commitment level?
- What was the scope or purpose of the Six Sigma project? Was the right problem statement identified and was Six Sigma the right approach?
- Who was the project manager, sponsor, or champion? Did they have appropriate authority level? Did they has sufficient resources or support?
- What was the success level measured against? Were the KPI's properly identified? Did the project address these metrics directly/indirectly?
- At what stage was the project deemed a failure? Was there a proper monitor and control phase? How long was data collected for the end state? Were there any changes made during the process?
- Was the project doomed from the start? Were the goals (scope, budget, schedule) realistic?
