Using the SIPOC Diagram - Template Included

Eldon Kao

What is it?
A SIPOC (Suppliers Input Process Output Customers) diagram is used during the Define phase of the DMAIC process as a high-level process map to help focus a continuous improvement effort. Although the SIPOC analysis is straight forward and simple, failure to perform this analysis in the early stages of continuous improvement may introduce confusion or disagreement regarding the scope and elements of the project among team members.

What’s Good and Bad about Failure Mode and Effects Analysis (FMEA) – Template Included

Eldon Kao

Failure Mode and Effects Analysis (FMEA) is a great tool for improving system reliability and reducing incidents of defects within a process. During the DMAIC process the FMEA can be employed to refine the focus of a six sigma project, rank contributing factors to root causes, and to identify corrective and preventive actions. The idea is to create a list of process steps and determine all potential failure modes associated with that step. These are then ranked in terms of Severity, Occurrence, and Detection which are multiplied to represent the Risk Priority Number (RPN). The resulting RPN is then used to prioritize efforts in regards to which problems to solve. The key to optimizing the use of any tool is to be aware its benefits and limitations. So here are a few in regards to FMEA:

Why You Should Write a Lessons Learned Report – Template Included

Eldon Kao

Six Sigma is a process like any other it is employed to improve. As such, it is also subject for continuous improvement. There are bound to be obstacles and challenges encountered throughout the weeks and months undergoing the DMAIC process. The purpose of documenting Lessons Learned is to catalogue issues in order to better predict and prevent similar incidents in future Six Sigma projects. On the other hand, there are likely many key successes during the project. The Lessons Learned is also a place to note desired results and perhaps develop a strategy to cultivate those behaviours. The effectiveness of the Lessons Learned document is limited only to the lessons recorded, therefore it is important to recognize lessons as they present themselves. The following events should prompt documentation:
